Duties and Responsibilities:

To have a key role in the management of patients with kidney disease, who often require a large number of medications.
To help with compliance aids for patients on various medications which serve as a prompt on when to take medications.
Expert in medications, we liaise with the doctors, nurses and dieticians to ensure patients get the right medications at the right dose.
We advise on drug interactions, dose adjustments tailored to renal patients and offer recommendations on the best treatment available for our patients.
To provide individually tailored medication counseling service for all our patients, thus ensuring that all patients are knowledgeable about their medications.
To be involved in the training and education of healthcare professionals with regards to medications specific to renal patients.
To be involved in discharge planning and dispense TTAs (to take away medications) for patients discharged from 10 South and 10 East in a timely manner.
To liaise with GPs to communicate changes made to medications during an in-patient stay.
To assist in the formulation and review of medication guidelines and protocols in accordance with the recommendations from national guidelines.
To look after the external dialysis units – organise the supply and delivery of medications for the units and the patients.
